Your Voice, Your Choice: Advance Care Planning Workshop

Hope Dementia Support Community Education Presentation will host a free interactive online workshop on completing an Advance
Directive from 6:30 to 8 p.m. March 28.

All participants will be guided through this simple, yet thought provoking, process of choosing a health care agent and putting your wishes in writing related to your health care in the event that you are unable to speak for yourself.
